Who is eligible: Business owners listed on the account and any phones they purchase and own are covered for accidental damage, mechanical failure, and theft.
This benefit provides insurance coverage for the labor/parts necessary to repair or replace a mobile phone in the event of a covered claim for Accidental Damage, Mechanical Failure, or Theft. All owners on the account and any phone they purchase or own are covered. This coverage has a benefit of up to $500 with a $50 deductible and has a limit of two claims per 12 month period. A 30-day waiting period is required before a claim can be filed for Accidental Damage or Mechanical Breakdown*. There is no waiting period for Theft claims.

Call or visit your financial institution and let them know you would like to file a claim prior to getting your phone repaired or replaced. They will verify your eligibility and provide you with a link to the online claims portal.
Go to the online claims portal within 90 days of the loss or damage of your covered device.
Enter all the required information on the electronic claim form.
A claims adjuster will review your submission. If no additional information is needed, you will receive an email with instructions and suggestions on approved repair/replacement facilities where you can take your device locally.
If there are questions about your submission, a claims adjuster will call you.
If your claim form is received prior to 3 PM Central Time, you will typically receive a response within 1 business day. If after 3 PM Central Time, you will typically receive a response within 2 business days.
